Which insurance companies are you in-network with?

Meadowlark Dental Care is a contracted Delta Dental Premier Provider, SunLife Financial (formerly known as Assurant) Provider, and a United
Healthcare Provider. Based on your insurance policy, there may be lower or reduced fees of procedures billed to you after your insurance has
paid their portion. Some exclusions may apply.

My insurance company is not listed as one you are in-network with. Can I be a patient?

Yes! We can bill all dental insurance companies regardless of our network status.

What is coinsurance?

Coinsurance is an insurance term that refers to the amount of money you, as the patient, pay out of pocket after you have met your deductible
and your insurance has paid their portion of the bill. Coinsurance will vary based on procedures, specific plans, and UCR (usual, customary and
reasonable) fees set forth by your insurance company.

My insurance says they cover 100% of preventive services, but it did not cover 100% of your fees! Why?

Great question! This has to do with the UCR (usual, customary and reasonable) fees that are set forth by your insurance company for each procedure.
Your insurance will pay 100% up to the UCR fee they have set, and the remaining difference between their fee and our procedure fee is your
responsibility. As an office, we have no control over what your insurance company deems are UCR fees and we may not be able to access their exact
amounts.

Do you accept Medicare?

Medicare does not have dental coverage unless a supplemental plan has been purchased. Please contact our office for additional questions.
